Join us for the "Go Your Own Way" Advanced Care Planning workshop on  April 23, 2024 from 10:00am-2:00pm.  

Advanced care planning is the process that enables individuals to make plans about their future health care.  It involves discussing and preparing for future decisions about your medical care if you become seriously ill or unable to communicate your wishes.  Advanced Care planning also ensures that family and friends are protected from having to make potentially life-changing decisions in the case of a medical emergency or from a chronic or terminal illness. Your plan helps ensure that you "go your own way", no matter the situation.

In this workshop, we will walk individuals and family members through the Advanced Care planning process.  These are not always easy conversations to have so our on-site experts are available to provide guidance, insight, and direction throughout this process. 

The workshop will kick-off with an informational session introducing the purpose and process of Advanced Care process.  Immediately following lunch, family group work sessions will take place.  This is a collaborative process that will require participation from both the individual creating the plan as well as the individual responsible for implementing the plan, should the need arise.

• Families and/or caregivers of aging or senior individuals

• Families and/or caregivers of individuals with chronic or terminal illness

• Families and/or caregivers of individuals that receive long-term care or are in a long-term care facility

• Families and/or caregivers that want to create their own Advanced Care Plan
